Please accompany all minors to this event or contact your Kids or Student Ministry Leaders.
Assumption of Risk & Liability Release
I voluntarily choose to participate in this activity and fully accept and assume all risks associated with participation.
I agree to follow all instructions provided by event leaders, guides, and safety personnel, including wearing a life jacket whenever required.
I release and hold harmless Rileyville Baptist Church, its staff, volunteers, leaders, officers, and representatives from any claims, liabilities, damages, or expenses arising out of participation in this event, except in cases of gross negligence or willful misconduct.
I understand that I may also be required to complete the venue's liability waiver before participating.
